CP Case Management
Community Partnerships offers Case Management services similar to what is offered with the Children Come First program, however, youth do not need to be a Dane County resident to be eligible. Community Partnerships can provide tailored case management services for any youth in need of support to be successful in their homes, schools, and communities.
What you can expect from Community Partnerships’ Case Management service:
0-30 days
- Intake to include:
- A strength-based Initial Assessment
- Child Behavior Checklist (CBCL) Assessment
- A comprehensive Trauma-Informed Assessment
- Development of a Crisis Plan, outlining child's triggers, warning signs, and crisis behavior, as well as identifying helpful strategies, approaches, and interventions to use to help de-escalate crisis situations.
- Development of a Plan of Care (POC), which is an individualized treatment plan with measurable goals, that outlines the services and supports the child needs to be successful in identified areas (school, home, community, etc).
- Development of a Child & Family Team--a network of support people for the child and family, including treatment professionals, school staff, community members, and natural supports.
- Weekly face-to-face meetings with caregiver.
- Minimum of one face-to-face individual meeting with youth (more as indicated).
- Coordination of weekly status reports from treatment/service providers and social worker.
- Initial Child & Family Team Meeting within the first 30 days.
- All documentation (progress notes, POCs, Crisis Plans, assessments, etc.) are updated regularly, kept confidential and maintained in a client chart.
31-60 days:
- Monthly Child & Family Team Meetings to assess child and family’s needs, update on status of treatment, review the POC and Crisis Plan, and discuss progress toward goals.
- Weekly face-to-face meetings with caregiver, at least one of which will occur in the home (more as indicated).
- Minimum of one face-to-face individual meeting with youth per month (more as indicated).
- Coordination of weekly status reports from treatment/service providers and social worker.
Beyond 60 days:
- Monthly Child & Family Team Meetings to update on status of treatment, review the POC and Crisis Plan, discuss progress toward goals, and plan for long-term support for the child and family post-CP involvement.
- Weekly contact with caregiver via phone, email, and/or in-person visits.
- Minimum of one face-to-face individual meeting with youth per month (more as indicated).
- Coordination of weekly status reports from treatment/service providers and social worker.
- Regularly scheduled updates to POC and Crisis Plan.
- CBCL and Trauma-Informed Assessment re-evaluation as indicated.
- A comprehensive Discharge Summary at the time of disenrollment.
The following are also available throughout enrollment:
- Access to 24/7 after hours crisis consultation and coordination
- Parent/Family Advocacy – support and assistance is available through the Parent Advocate
- ArtSpeak groups – expressive arts group is available to all participants at no additional cost
